It typically involves identifying needs, locating appropriate resources, coordinating services, making referrals, and following up to support continuity of care. Case management does not replace medical treatment — it helps people find and stay connected to the right services.
Does HAPPY ABODE RESIDENTIAL LIVING provide medical treatment or therapy directly?
No. Case management focuses on coordination, navigation, and referral — connecting individuals with appropriate providers and community resources. We are not a substitute for medical care, therapy, or emergency services. If you are experiencing a medical emergency, call 911.

Is there a cost for case management services?
